(for Children, Young Teens, and Bridge Teens)
This agreement is between (parent or legal guardian) and IMPACT International and Prepare, Inc., their employees, contractors and affiliated persons and organizations (collectively referred to as “PREPARE”). In consideration for allowing my child(ren) to participate in the Prepare Self-Defense Workshop (Workshop), I make the following statements and promises:
- I am the parent or legal guardian of registered child(ren).
- I am voluntarily letting my child(ren) take the Workshop with knowledge that they will be participating
in simulated attack scenarios involving physical contact and I agree to accept any and all risks of
injury to my child(ren).
I agree that I, my heirs, legal representatives and assigns:
- Will not make a claim against PREPARE for any injury or damage resulting from my child(ren)’s
participation in the Workshop, including but not limited to exposure to or resulting illness from Covid 19 or variants, and - Will release and discharge PREPARE from all claims or demands arising from injury or damage to my
child(ren) caused by my child(ren)’s participation in the Workshop.
I promise to defend, indemnify and hold PREPARE harmless from any claims and actions by third
parties alleging injury from my child(ren)’s use of the techniques learned in the Workshop.
I understand that participation in the Workshop in no way guarantees my child’s (children’s) safety in
a real-life situation.
I have not requested nor received any warranties as to the effectiveness of the Workshop.
In the event that my child(ren) is injured or becomes ill during the Workshop, I hereby grant
permission to PREPARE to call a physician or take him/her to a hospital, if necessary, and at my
expense. It is understood that this will only be done if I am unavailable or there is insufficient time to
reach me.
I have carefully read this agreement and fully understand its contents. I am aware that this is a
Release of Liability and a contract between myself and PREPARE and sign it of my own free will.
